Jack’s Place is committed to providing a home away from home for you and your family. This home is available so that you may maximize your time spent daily with your hospitalized family member. Additional guests joining you are allowed at the discretion of the House Manager. One room is offered per family.

Jack’s Place can accommodate 8 families. For that reason, we are certain that you can appreciate the need for specific regulations which we hope will ensure the comfort of our residents. We ask that you be considerate of your fellow residents, particularly with regards to privacy, cleanliness, visitors, smoking and noise. Treat Jack’s Place as if it were your own home in a residential neighborhood.

Jack’s Place is a unique facility supported by its volunteers. Therefore, we also rely on your willingness to share in the responsibilities of upkeep. In addition to taking care of your own needs, feel free to lend a hand when you have the opportunity, but cleaning up after yourself and your family members comes first.

The mission of Jack’s Place is to provide a “home” for the families of out of town patients served by Ellis Medicine and Sunnyview Rehabilitation Hospitals in Schenectady while their loved ones are rehabilitating.

AROUND THE HOUSE

FIRE/SMOKE ALARM

The smoke-fire alarm system is operational 24 hours a day. If the alarm sounds, proceed to the nearest exit. Please gather in the back yard for further instructions from staff.

PARKING

Parking is available in the Ellis Parking Garage located across the street and in the Sunnyview Parking Lot within walking distance. During winter inclement weather, please utilize the Ellis Parking Garage so that the Sunnyview Parking Lot can be properly plowed. Drop-off and Pick-up is available in the circular drive in front of the house on Rosa Road.

YOUR BEDROOM AND BATHROOM

Sheets must be used on all beds. Sheets, blankets and towels are provided in each room. You are responsible for laundry and keeping your room and bathroom clean and neat during your stay. Please be sure to turn off lights and air conditioner when leaving your room. Cleaning caddies are available in the first floor broom closet and in the third floor storage closet. A vacuum is available for your convenience.

KITCHEN

Guests are responsible for maintaining the cleanliness of the kitchen and dining room. For health reasons, dishes and utensils must be washed in the dishwasher only. Pots and pans should be washed by hand, dried and put away. Empty trash when full.

You may use the refrigerator to store food. We ask that you label each item with your name and the date. Upon departure, please discard your family’s perishable food items. A pantry shelf is provided with your room number on it for storing food as well.

HOUSE MEALS

A Thursday schedule of evening meals generously prepared for our registered guests by volunteers or provided by a local restaurant will be posted. Meals are usually ready at 6pm. Our guests may find volunteers in the kitchen on a Friday morning baking such treats as cookies, brownies, muffins, cupcakes and pies.

LAUNDRY

Two washers, two dryers and laundry detergent are available for use in the basement. An iron and ironing board are available for use in the laundry facility only.

COMMON AREAS

The living room features a large screen TV and multiple seating areas. A large screen TV, family computer and printer, age appropriate books, board games, coloring books and puzzles are located in the basement family room. Our backyard, patio and gazebo are also available for your use. Appropriate attire is required in all common areas.

A courtesy telephone is located in the kitchen. The Phone Number is 518-952-7648.

Guest Guidelines

SECURITY

For security purposes, keep your room locked at all times. It is recommended that you refrain from leaving valuables or money in the house. We are not responsible for lost items. Be sure that all exterior doors are closed securely when you enter and exit the house. Please note that for your safety and security, a recording is entered each time the proximity reader is used to open a door and all common areas inside and outside the house are under 24 hour, 7 day per week surveillance. TO FURTHER ENSURE THE SAFETY OF ALL GUESTS, SEX OFFENDER INQUIRIES WILL BE COMPLETED ON ALL GUESTS THROUGH THE NEW YORK STATE DIVISION OF CRIMINAL JUSTICE SERVICES SEX OFFENDER REGISTRY. INDIVIDUALS WITH A HISTORY OF CRIMINAL SEXUAL OFFENSE WILL NOT BE PERMITTED AT JACK'S PLACE.

Report any breakage or malfunctioning equipment to the Ellis Security Staff. We would like to ensure everything is in working order and keep everyone safe and sound.

Help us conserve energy. We would appreciate it if you would turn of the lights and the television when they are not in use.

PETS

No pets of any kind are allowed on the premises.

CONDUCT

Please maintain a respectful, quiet environment. The use of alcohol, illegal drugs, tobacco products, weapons and vulgarity is not permitted. Violation will lead to eviction from Jack’s Place.

Quiet hours are between 9:00 PM and 9:00 AM. Jack’s Place is shared by families and children, so please wear modest attire in public areas. No bare feet in public areas please.

EATING AND DRINKING

Eating is permitted in the kitchen dining area only. Only baby bottles are permitted outside this designated area. Do not store or take food or beverages into the bedrooms or other common areas. A pantry shelf is provided with your room number on it.

CHILDREN

Children under the age of 18 must be supervised at all times in Jack’s Place. Guests are responsible for the supervision and safety of their children. Please clean up after your child is finished playing and eating. We ask that you and your children respect quiet hours from 9:00 PM to 9:00 AM.

VISITORS

Visitors are welcome with prior approval only between the hours of 9 AM and 9 PM and are limited to the common areas of the house.

LENGTH OF STAY

Families may stay at Jack’s Place for up to 15 days. As your stay approaches 30 days, your situation will be reviewed and your stay may be extended for valid medical reasons.

ILLNESS

Should you come down with any infectious/contagious illness during your stay, please inform case management immediately. For the well-being of patents with fragile health, we will ask you to make alternate lodging arrangements for the duration of your illness.

ROOM FEES/REQUIRED DONATION

We never ask our guests to pay for the services provided. If a guest asks for a suggested donation amount, we will request $10 per room night provided.

CHECK OUT

Before leaving, wash and dry your sheets and towels. Make the bed, dust, vacuum, clean glass dresser tops and the bathroom to prepare for our next family. Close and lock the windows. Empty the trash from your room and dispose of all your perishable food items in the kitchen. Return room key(s) to Ellis Security or place in the drop box on the wall in the kitchen at Jack's Place when you sign out.
